Families with Power of Attorney use Relatives Gateway to access their loved ones’ care plans and any aspect of their care including meals, wellbeing and activities. This two-way portal provides access to your relative’s day-to-day notes and information about any aspect of their care including meals, daily routine and wellbeing activities. You can respond by making comments, sending messages, asking questions and uploading photos.

It’s a very straightforward system. Staff use hand-held devices loaded with apps to record actions taken at the point of care (care notes) which are then uploaded centrally. Such technology removes the need to find each resident’s documents and write up notes manually using pen and paper, which can also get lost. This way, each resident’s care notes are safe and secure, and can be shared with relatives via Relatives Gateway. As a result, less time is spent on administration and more with residents.

The system also includes a comprehensive shift handover process to communicate with care staff easily, and track if that information has been received. The families’ input into what’s been documented helps drive up documentation standards, while the openness and transparency facilitates greater trust and healthier working relationships between families and the homes. It’s secure, too, as access can only be granted upon each resident’s approval, and it is also easy to track if care plans remain tailored to individual needs.

During the pandemic – especially at times when the home was closed to visitors – the portal really came into its own, helping users to stay in touch with their loved one. The sharing of photos and messages positively impacted our residents’ mental and physical wellbeing, as well as that of their families. Users were also able to track their relative’s care and general mood after each interaction with a carer, enabling them to play an important part in their care, and be reassured that they were safe and enjoying the best possible quality of life and wellbeing at a very difficult time.

The system also provides at-a-glance information about each resident’s vaccination status and whether or not they have contracted Covid-19. Through the use of risk assessments and daily checks, this important information was speedily made available to the families via the Relatives’ Gateway.

To gain access to Relatives Gateway relatives need a Power of Attorney or permission from their loved one. If you are interested in finding out more, please contact the home manager.
